Notes Regarding Submission

All applications must be submitted online. Attachments are required.

Please note that all deadlines are 11:59 PM EST (Washington, DC) on the day of the deadline.

The final page of the attachment templates should be signed and returned to AHAF as specified in the instructions. AHAF does not require signatures to be submitted by the deadline. Please submit the signature pages to AHAF within two weeks of the application deadline.

Please note that each program announcement below is associated with an application ‘Template and Guidelines’ document. The template portion of this document should be filled out using your word processor. In practice, this file is what will be supplied to your reviewers. Once completed, please upload this file to the online submission website associated with the program to which you are applying. On the submission website, you will be posed some questions related to your proposal, responses to these questions will be used for administrative purposes at the foundation.

Please ensure that you are using forms appropriate to the current fiscal year (FY09)

Access an "In Progress" Proposal Notice: An error during unveiling of this website may affect some National Glaucoma Research (NGR) applicants. During October 6-10, the website incorrectly linked to a prior version of the application guidelines and template. The first page of the current version (attached) should show October 27, 2008 as the deadline for submission, and should be approximately 35 pages long including instructions, templates, and worksheet. If this error affects you, we do apologize for the inconvenience.
